Instructions for Paperwork

General Rules:

  1. Students should always follow the graduate school deadlines.
  2. When communicating with the Graduate Program Assistant via email, students should always copy the Graduate Program Director.

Paperwork for Master’s students who are doing a thesis

  1. Register for 6 credits of Math 599.
  2. Form a thesis committee. Students need to go to the student portal and follow the instructions there.
  3. After defending the thesis, students need to inform the Graduate Program Assistant (and copy their thesis advisor and the  Graduate Program Director) so that she can complete the Thesis Final Defense Form.
  4. Afterward, revise and submit their thesis to ProQuest.
  5. Afterward, inform the Graduate Program Assistant (and copy their thesis advisor and the  Graduate Program Director) so that she can initiate the ETD Thesis Committee Acceptance Form.

Paperwork for Master’s students who are doing a project

  1. Register for 3 credits of Math 598 devoted to a project supervised by an associate or full member of the graduate faculty in mathematics.
  2. After the project is completed, the student should write a project report using LaTeX. After approved by the faculty supervisor, the student must email the pdf file of the project report to the Graduate Program Assistant (and copy the faculty supervisor and the Graduate Program Director).

Paperwork for PhD students

  1. To form a dissertation committee, students need to go to the student portal and follow the instructions there.
  2. After a student passes both qualifying exams or the comprehensive exam, the graduate program assistant need to report the results to the graduate school using the Doctoral Qualifying Exam Form.
  3. Students need to have a PhD plan of study, which is a list of courses of at least 48 credits that count toward the PhD degree. They should discuss their courses with the graduate program director and seek approval. If approved, they need to email the graduate program assistant (and copy their advisor and the graduate program director) to initiate the PhD Plan of Study Form.
  4. After passing the dissertation proposal, the student must email the graduate program assistant (and copy their advisor and the graduate program director) to initiate the Doctoral Admission to Candidacy Form.
  5. At least two weeks before the scheduled defense, students need to publicly announce their defense through the student portal.
  6. After passing the dissertation defense, the student must email the graduate program assistant (and copy their advisor and the graduate program director) to initiate the Doctoral Final Defense Form.
  7. After the dissertation is approved by the dissertation committee and submitted to ProQuest ETD (electronic thesis and dissertation), the student must email the graduate program assistant (and copy their advisor and the graduate program director) to initiate the Committee Acceptance Form for Electronic Dissertation.
